A Liver Function Test — also called LFT or Liver Panel — is a group of blood tests that check how well your liver is working. It is not a single test but a battery of 6–8 tests done together, each measuring something different about liver health.

Doctors order an LFT when:

  • You have symptoms like yellowing of skin or eyes (jaundice), fatigue, or abdominal pain

Understanding Each LFT Value — In Plain Language

ALT (SGPT) — The Most Important Liver Marker

ALT, also called SGPT in Indian lab reports, is the most liver-specific enzyme in this panel. It is produced almost exclusively in liver cells.

When liver cells are damaged or inflamed, ALT leaks into the bloodstream — and your blood test picks it up.

High ALT means: liver cells are under stress or being damaged. Common causes include fatty liver disease (extremely prevalent in urban India), viral hepatitis (Hepatitis B and C), alcohol consumption, or certain medications including TB drugs and painkillers.

Mildly elevated ALT (56–100 U/L): often seen in fatty liver or after a heavy meal with alcohol. Usually reversible with lifestyle changes.

Significantly elevated ALT (above 200–300 U/L): suggests more serious liver injury — hepatitis, toxicity, or liver disease. Needs prompt medical evaluation.

AST (SGOT) — Liver and Beyond

AST, or SGOT, is found not just in the liver but also in the heart, muscles, and kidneys. So a high AST alone does not always mean liver trouble.

Doctors look at the AST:ALT ratio to interpret results:

  • Ratio close to 1 (both elevated equally) → likely viral hepatitis or fatty liver
  • Ratio above 2:1 (AST much higher than ALT) → often points to alcohol-related liver disease

ALP — The Bile Duct Signal

Serum Albumin and Total Protein measure the liver's ability to produce proteins — one of its key functions.

Low Albumin is a sign of chronic liver disease (not acute). When the liver has been damaged over a long period — cirrhosis, chronic hepatitis — its ability to manufacture albumin declines.

Low albumin can also occur due to malnutrition — relevant in India where protein intake in many households is below recommended levels.

Fatty Liver Disease

This is the most common reason for mildly elevated ALT and AST in urban India today. A diet high in refined carbohydrates, maida, sugar, and fried food — combined with low physical activity — leads to fat accumulation in the liver.

Fatty liver often shows no symptoms but quietly progresses if ignored. The good news: it is largely reversible with diet and exercise in early stages.

Hepatitis B and C

India has a significant burden of Hepatitis B — transmitted through infected blood, contaminated needles, or at birth. Many people carry the virus without symptoms for years while liver damage silently progresses.

TB Medications (Anti-TB Drugs)
