Troponin (T or I)

Troponin is a protein found inside heart muscle cells. When heart cells are injured — most commonly during a heart attack — troponin leaks into the bloodstream. High-sensitivity troponin tests can detect even tiny amounts, allowing earlier diagnosis of heart attacks. Troponin is the gold standard biomarker for myocardial infarction (heart attack).

What Low Levels Mean

Undetectable or very low troponin is normal and indicates no heart muscle damage. A normal troponin in the setting of chest pain makes heart attack very unlikely (though serial measurements over 6–12 hours are needed to fully exclude it).

What High Levels Mean

Elevated troponin indicates heart muscle damage. The most critical cause is acute myocardial infarction (heart attack). Other causes include myocarditis (inflammation), pulmonary embolism, heart failure, sepsis, and kidney failure (impaired clearance). Any troponin elevation with chest pain is a medical emergency.

How is troponin used to diagnose a heart attack?

Troponin rises within 3–6 hours of heart muscle damage and peaks at 12–24 hours. A rising and/or falling pattern on serial measurements (tested at 0, 3, and 6 hours) is characteristic of heart attack. A single elevated reading without a rise/fall pattern may indicate chronic heart damage rather than acute heart attack.

Can troponin be elevated without a heart attack?

Yes. Myocarditis (viral heart inflammation), pulmonary embolism, heart failure, sepsis, strenuous exercise (marathon running), and chronic kidney disease can all elevate troponin. The clinical context — symptoms, ECG, and the pattern of troponin change — determines the diagnosis.

What should I do if my troponin is elevated?

Any troponin elevation with chest pain, shortness of breath, or other cardiac symptoms is a medical emergency — go to the nearest emergency room immediately. If troponin is incidentally found to be mildly elevated without symptoms, your doctor will investigate the cause and may order serial measurements.
